Annual Dancer Registration, Access & Participation Form 2026/27

Thank you for being part of Kate Stanforth Academy of Arts.

We ask all dancers to complete this form once each academic year. It helps us understand your access requirements, support your participation, keep appropriate emergency information and understand the communities our work reaches. Some questions are required for participation, while others are optional and help us evaluate and improve our work and demonstrate our impact to funders.

Choosing not to answer optional equality or monitoring questions will never affect your ability to participate in the Academy.

We usually only use your address for emergencies but we now have an opportunity to use this information to help us understand where our dancers are based, the geographical reach of KSAA and where there may be demand for accessible dance opportunities.

We may use location information to create maps, statistics, funding reports and impact information about our community. Public-facing maps and reports will use anonymised or aggregated information wherever appropriate. We will not publish your home address, full postcode or precise individual location on a public map.

Section 4 - Terms and Conditions

Dance and physical activity involve inherent risks. KSAA encourages dancers to participate in ways appropriate for their own bodies, use adaptations, take breaks and communicate access requirements where helpful. KSAA teachers provide dance instruction and inclusive movement options but cannot provide individual medical advice. Participants should stop participating if they become unwell or experience unexpected pain or symptoms and seek appropriate assistance where necessary.

Section 5 - Photography & Filming

Kate Stanforth Academy of Arts (KSAA) regularly photographs and films classes, workshops, performances, events and projects to celebrate our dancers, showcase our work and promote inclusive dance. Images and video may be used across KSAA's website, social media, newsletters, promotional materials, funding applications, reports and presentations.

Content may also be shared through Kate Stanforth's professional and social-media channels and, where relevant to the activity, by KSAA teachers, guest teachers, creatives, project partners, venues, funders and collaborators on their professional channels. This may include photographs, short-form video, reels and other content showing the work they have delivered or supported with KSAA.

From time to time, KSAA may work with brands, ambassadors, influencers, content creators, press or other media organisations to increase awareness of our work and the representation of disabled dancers. Where a proposed use is substantially different from our usual Academy promotion, particularly significant external media or commercial campaigns, we may contact you to request additional permission.

Media consent is entirely optional. Choosing not to give permission will not affect your/the dancer's ability to participate in KSAA activities. You can contact us at any time to update your preferences for future use. We will take reasonable steps to stop future use following a change of preference, although we may not always be able to withdraw material that has already been published, printed, distributed or shared appropriately.
